Use these reports to present your findings to the customer/homeowner in a methodical way, making it easier for them to understand their chimney and where defects are located. These forms do not auto-fill with your findings from the checklist.
These reports have two main uses:
To be a short report when you come across a chimney or fireplace that doesn't need extensive repairs.
example: A woodstove, that has dirty glass or needs the gasket replaced
example: the annual inspection of a masonry fireplace that's used twice a year and the owners have good burning practices.
To be a placeholder for the homeowner/customer while someone (you or the the office) puts together a work proposal.
example: A furnace with it's vent/chimney liner running part way up a masonry chimney. As a result, condensation has attacked the unlined half of the chimney, and the masonry is falling apart. The customer wants you to rebuild from the ground up.
